The Right Dose: From Phase I to Clinical Practice

چکیده

To realize the full potential of promising new anticancer drugs, it is paramount importance to administer them at right dose. The aim this educational article provide several opportunities optimize drug dosing, focusing on oral targeted therapies. First, therapeutic monitoring can exposure in individual patients, if optimal concentration known. This approach particular interest regard kinase inhibitors with high interindividual pharmacokinetic variability. If related response, then potentially feasible, although clinical utility has not yet been established. Other approaches reduce variability include administration more frequent, smaller doses and under prandial conditions. However, for many labeled dose demonstrated be dose; such agents, vast majority patients may receiving excessive doses, which results toxicity. Furthermore, lower off-label both medical financial These strategies should applied from registration studies practice, goal better optimizing treatment.

Principles from clinical trials relevant to clinical practice: Part I.

The first 2 parts of this review discussed lessons learned from cardiovascular clinical trials about the design and interpretation of clinical trials. In the next 2 parts, we will attempt to apply these lessons to decision-making in clinical practice. We undertake this effort with trepidation, recognizing that translating research findings into the care of an individual patient is a frontier th...

Dose Escalation Methods in Phase I Cancer Clinical Trials

Phase I clinical trials are an essential step in the development of anticancer drugs. The main goal of these studies is to establish the recommended dose and/or schedule of new drugs or drug combinations for phase II trials. The guiding principle for dose escalation in phase I trials is to avoid exposing too many patients to subtherapeutic doses while preserving safety and maintaining rapid acc...

Dose escalation with over-dose and under-dose controls in Phase I/II clinical trials.

To save valuable time and resources in new drug development, Phase I/II clinical trials with toxicity control and drug efficacy as dual primary endpoints have become increasingly popular. Escalation with over-dose control (the EWOC) is a Bayesian adaptive Phase I clinical trial design that can accurately estimate the maximum tolerated dose (MTD) level and control the probability of overdosing p...
